Those who enroll in Live At Home secure the benefits of escalating levels of care designed to provide essential health and personal services when and where needed.
Through each level of care (described below), as a Live At Home member you are guided by a professional Care Manager, who becomes a consultant, confidant and friend as the years go by. The role of your Care Manager is to respond to your changing needs and to organize resources to help you remain healthy, active, and living at home.
Level One – Wellness By Design
Designed to meet the needs of members when they are healthy and active, features include:
• An emphasis on improving wellness and health.
• Includes a baseline health assessment.
• Information and educational programming on nutrition,
exercise and medical treatments.
• Referrals to trustworthy services for everything from pharmacy delivery services, physical therapy,
house call physician practices, nutritional planning, to home maintenance.
At the Wellness Level, you will become acquainted with your Care Manager, a multi-disciplinary professional, who is your link to the expertise available throughout the Alexian Brothers' network of services. Your Care Manager guides you to take advantage of program benefits and organizes resources in the community on your behalf. Your Care Manager stays in contact with you throughout the year, and as often as you prefer, to answer questions or to consult you on healthcare dicisions and changing needs.
Level Two – Intervention By Design
Designed to respond to changing needs, services at this level include:
• All the Wellness benefits of Level One.
• Increased support from your Care Manager and staff in response to your needs.
• Support with activities of daily living, coordinated by your Care Manager, provided by trained professionals, at no extra cost, to help with routine housekeeping, laundry, food preperation, and medication reminders.
Live At Home provides essential transportation if you are not able to drive safely, or if you have a physical condition that prevents driving.
Transportation for essential chores include:
• Medical appointments • Dental appointments • Grocery shopping
• Visiting the pharmacy • Visiting the bank
Level Three – Support By Design
A more intensive level of service designed to respond to your changing needs.
• All the benefits of Levels One & Two.
• Intensive support from your Care Manager and staff.
• Personal care assistance in your home from trained nursing assistants, at no extra cost.
There is a continued emphasis on keeping you healthly and active, what you can still do on your own and for what cannot be done alone, your service levels are designed to meet your changing needs. If more intensive in-home assistance is needed, Live At Home provides you a personal emergency response system to ensure your safety.
Level Four – Care By Design
Designed to provide intensive care and for most people, without the nursing home.
• Access to the benifits of Levels One, Two and Three.
• Options: If you are "nursing home eligible", you have an opportunity to remain at home and receive care or relocate to an Alexian Brothers facility.
• If qualified, enrollment in Program of All-Inclusive Care of the Elderly (PACE), at no additional charge.
• 75% of your cost of moving into an Alexian Brothers nursing facility.
Live At Home is committed to providing you all the tools you need to ensure you remain in your home for as long as possible. As a member of Live At Home, you may request that Live At Home continue to provide service to you in your own home up to the amount equal to 75% of the cost of nursing facility care.
As a member you have priority status with any of the following Alexian Brothers facilities.
