Alexian Village Skilled Nursing & Rehabilitation Center

Alexian Village's Health and Rehabilitation Center provides the services associated with a nursing home, but we do so in a home-like atmosphere, and with a goal to make sure the residents reach and enjoy the highest level of independence. 

The Health and Rehabilitation Center provides twenty-four hour skilled nursing, rehabilitation services, supported recovery following hospitalization, as well as memory care services for residents with Alzheimer's and other dementia, and ongoing medical care and observation for more chronic conditions.  

Our professional skilled nursing team and staff work passionately to provide the most compassionate care. Under the direction of the resident's physician, the staff has the experience and expertise to effectively manage complex health care needs. 

Alexian's Health and Rehabilitation Center feature generously sized accommodations in a home-like atmosphere.  There is ample space in several lounge areas nearby to have quiet visits with family and friends.  Residents are offered a menu with a wide variety of meal choices to accommodate personal preferences.  Therapeutic diet plans are also available.

The dedicated team of skilled nursing and rehabilitation specialists in the Health and  Rehabilitation Center is comprised of physical, occupational, speech and recreational therapists.  We also specialize in pain management, balance training, Lymphedema Therapy, Vital Stim Therapy, and low vision screening. 

 
Memory Support is a specialty unit designed to provide a secure, comforting and supportive environment for residents with Alzheimer's disease or other related dementias.  The caregivers have advanced training that enable them to respond appropriately to the special physical, emotional, social and sensory needs of the residents.
