The Health Center provides twenty-four hour skilled nursing, rehabilitation services and supportive care during recovery from an acute illness following hospitalization.  Ongoing medical care and observation for more chronic conditions is also available.  The Health Center professional staff diligently works to help residents achieve their highest level of independence. Under the direction of the resident’s physician, the staff of the Health Center has the experience and expertise to effectively manage complex health care needs including:
Skilled Nursing Rehabilitation Infusion Therapy Enteral Feedings Pain Management Extensive Wound Care Diabetic Management Peritoneal Dialysis Cardiac and Pulmonary Disorders     and other medical conditions	The Health Center works closely with the resident’s personal physician to design a complete plan of care that will help the resident achieve established goals.  Nursing staff, therapists, dieticians and social works all work together to provide the most comprehensive care available.
	The Health Center features generously sized accommodations in a home-like atmosphere.  There is ample space in several lounge areas nearby to have quiet visits with family and friends.  Residents are offered a menu with a wide variety of choices to accommodate personal preferences.  Therapeutic diet plans are also available.
The dedicated team of rehabilitation specialists in the Health Center is comprised of physical, occupational, speech and recreational therapists.  These disciplines work directly with the nursing staff to create a program specific to each resident’s needs.  These individualized treatment plans help the resident to achieve maximum functional status.  Residents and their families are key members in this team approach.  Team meetings are arranged with the family’s schedule in mind.  Rehabilitation is especially  beneficial following orthopedic surgery, strokes, heart attacks, prolonged illness or frequent falls at home.  The Health Center consistently achieves positive outcomes in a relatively short period of time.
Memory Support is a specialty unit designed to provide a secure, comforting and supportive environment for  residents with Alzheimer’s disease or other related dementias.  The care givers on the memory support unit have advanced training that enable them to respond appropriately to the special physical, emotional, social and sensory needs of the residents.
The Memory Support Unit features an environment that is welcoming and supportive.  This unit consistently offers a familiar frame of reference which reduces confusion and offers freedom to move about in an unrestricted space.  Unique programs have been implemented to promote individual resident capabilities. The Memory Support Unit has interdisciplinary team members who work closely  together to create individualized approaches that allow comprehensive care of these residents.
