Alexian Brothers Community Services (ABCS) is a community-based program whose purpose is to serve the frail elderly residents of Hamilton County. Alexian Brothers Community Services is part of the national Program of All-inclusive Care for the Elderly (PACE), which is based on the successful San Francisco model of long-term care for the elderly known as On Lok. Started locally in 1998, PACE is a jointly sponsored project of ABCS and the Tennessee Bureau of TennCare.
Alexian Brothers Community Services enables its participants to remain independent in their own homes and communities through a unique system of managed care.
Through a multi-disciplinary team made up of physicians, nurses, medical social workers, health aides, transportation providers, rehabilitation therapists, dietitians and others, an exclusive plan of care is determined for every participant with a preventive approach. Progress is constantly monitored for each participant to keep problems to a minimum and help avert health catastrophes.
Once a participant is enrolled in the program, Alexian Brothers Community Services provides all necessary health care services. Most care is delivered in an adult day health care center setting at 425 Cumberland Street, preserving the participant’s community residence, family relationship, and lifestyle.
Individuals who:
- are at least 55 years of age or older;
- live in Hamilton County, Tennessee;
- are assessed by the PACE multi-disciplinary team
as being appropriate for PACE Program services; and
- are certified by TennCare as eligible for nursing home level of care.

All services must be approved by the PACE team or doctor and desired by the PACE participant or family caregiver.
