Alexian Brothers PACE Program
An alternative to nursing home care quality health care while remaining in your home
The goal of the PACE Program is to provide the frail elderly with a financed health, medical and social services program and the independence to remain at home Qualified recipients will receive:
- A comprehensive program of Medicare/Medicaid at no expense to dual qualified Medicare and Medicaid participants (No fees for health services, co-payments or deductibles)
- A medical/health care program that is tailored to the needs of each participant
- A multi-disciplinary team of health care professionals that coordinate and work together on a personalized medical/health program for the participant – individualized care
- At-home health care services, personal care when needed
- Transportation to and from the Alexian Brothers PACE Adult Day Health Center
- The independence to live at home and still be well cared for
- PACE Adult Day Health Center (games, activities, socialization, hot lunch and more)
- In-house spiritual care services and programs

God's Hospitality Changes Lives
Personable, friendly, engaging and always straight forward in his convictions. The Alexian
Brothers and all who knew him will miss Brother Anthony who passed away April 29. His long life
afforded him the opportunity to touch the lives of many people. And touch them, he did. While at
Alexian Village of Milwaukee, he was known to say, "God's hospitality changes lives." It has
since become a popular motto there and a sign with its message graces the entrance to Alexian's Chapel of the Holy Spirit. God bless Brother Anthony for his ministry and the Alexian Brothers for providing him with a full life.
